These services assist persons with ongoing assessment, planning, referral, service coordination and monitoring of multiple needs. MH-TCM is provided to adults determined to have a serious and persistent mental illness (SPMI). Covered Adult MH TCM services include but are not limited to:

  • Assistance to obtain and access needed services
  • Coordination with other agencies to obtain services for a person
  • Completion and regularly review the written functional assessment for the person
  • Development and regularly review the written individual community support plan for a person based on both a functional and diagnostic assessment of the person and incorporate the individual treatment plans
  • Participation in case conferences, administrative reviews, staffing and informal conferences
  • Development of goals, service plans, written service agreements and routine case supervisory activities
  • Contact and other monitoring or communication with the person, family members or other relevant people about the status of the person, the Individual Service Plan or progress in achieving the goals of the service plan
  • Planning, assessment, record keeping and documentation associated with the service coordination of a case
  • Assessment and evaluation of the effectiveness of services received by a person
  • Discharge planning process monitoring for a person being discharged from residential treatment
