Elderly Waiver (EW) is for people over the age of 65 years who require the level of care provided in a nursing facility and choose to reside in the community (DHS Aging and Adult Services Division administers the EW program). Alternative Care (AC) for people over age 65 years who are at risk of nursing home placement (DHS Aging and Adult Services Division administers the AC program) Care Coordination includes activities that help people access, coordinate and monitor needed services as they relate to the person’s assessed needs and preferences. Needed services include Educational, Medical, Social, and Vocational.
Case management activities may be provided to or arranged for a person. Activities may include but are not limited to the following:
- Coordinate services
- Develop a service plan based on people’s assessed needs and preferences
- Evaluate and monitor services identified in the service plan
- Help people access needed services
- Help people identify potential service providers
- Inform people or their legal representative of service options
